What Makes Governed Knowledge Different

Governance is not a feature bolted on after the fact. It is structural — embedded in how knowledge is created, stored, deployed, and referenced.

Evidence Provenance & Stable Identity

Each source document is assigned a stable identity hash (source_identity_hash) at ingestion. This ensures that evidence can be traced unambiguously — even if the source is updated, moved, or re-published — and that the exact version used for rule derivation is always recoverable.

Rule Versioning & Content Hashing

Every clinical rule carries a version identifier and a content hash. If the rule logic changes, the hash changes — making it immediately detectable whether a finding was produced by the current rule or a prior version. Historical findings remain reproducible against the rule version that generated them.

Human Approval at Every Stage

No evidence enters the knowledge base without human review. No rule is deployed without formal approval. No clinical finding is presented as final without provider oversight. Governance is not automated — it is a human process supported by structured workflows.

Deployment Safety Interlocks

Rules move through defined stages before reaching the clinical environment. Deployment is gated by approval status, version consistency checks, and activation controls. A rule can be versioned and approved but not yet active — separating knowledge availability from clinical use.
